    Exam Request and Pet History Form

  • We require a $69 exam deposit and a Pet History form to schedule an exam. Submitting this form will charge your card for the deposit plus a 3% convenience fee.

    The $69 deposit only covers the charge for an exam and does not cover any tests, bloodwork, or vaccinations completed during the appointment.

    After you submit this form, our schedulers will review it and select the earliest appointment date possible. You will receive a text message and email with the appointment date and time.

  • The Alaska SPCA Clinic is not an urgent care. We have limited availability for sick pet exams. To inquire about availability and scheduling, please contact our Customer Help team at (907) 562-2999 or customerhelp@alaskaspca.org.

  • Note: Pre-dental and senior spay/neuter exam appointments WILL require diagnostic bloodwork. The charge for bloodwork is determined during the exam and payment will be due on the appointment date.

    Our clinic has limited exam availability, and our Customer Help team will make sure we have availability to schedule your sick pet before collecting an exam deposit.

  • Notice: We are fully booked for WELLNESS EXAMS for the next 90 days. Check back at the beginning of the month. DO NOT PROCEED WITH THE REST OF THIS FORM.

  • Health and History

  • If your pet is currently bleeding, in obvious pain, or has been involved in some kind of accident (stepped on, fell from a high place, etc.), your pet may be experiencing a medical emergency and needs urgent attention.

    In cases of an emergency, please take your pet to an urgent care clinic.

    In Anchorage, the urgent care clinics are:

    • Midnight Sun Animal Hospital on Tudor Road
    • Pet ER on the corner of Lake Otis Pkwy & Dowling Road

    In the Mat-Su Valley please go to Tier 1 on the frontage road of the Parks Hwy heading into Wasilla.

  • Exam Deposit

    The $69 deposit only covers the charge for an exam and does not cover the charges for any tests or bloodwork completed during the appointment.

    ONLY SUBMIT THIS FORM ONCE FOR EACH PET.

    If you need to change service selections, cancel, or reschedule after submitting the request, contact our Customer Help team. Notice of at least one business day prior to the appointment is required to receive a refund for cancellation.
